We’re seeking a committed Community Nurse to join our team and help improve the lives of older adults living independently in the community. In this rewarding role, you’ll draw on primary health care principles to deliver holistic, person-centred care to older adults experiencing chronic conditions and complex needs. Working as part of a multidisciplinary team, you'll provide early intervention, health education, and recovery-focused support within clients' homes and community settings. Day To Day This Role Looks Like Providing direct clinical nursing care to our clients in their homes in line with best practice nursing proceduresConducting clinical assessments, dressings, medication assessment and catheter changingDeveloping a holistic nursing care plan in collaboration with client, carer, Care Advisor and Allied health where relevantProviding safe, quality nursing practice to clients with specific needs and/or complex needs eg.
