Total Veteran Healthcare is seeking a highly skilled and compassionate Coordinated Veteran Care Registered Nurse to join our dedicated team of healthcare professionals. This is an exciting in-clinic role.

About the Role & Responsibilities

As a Coordinated Veteran Care Registered Nurse, you will be integral in delivering a veteran-centric approach, working closely with veterans to manage their health care plans, ensure they receive the highest standard of care, and support them in living healthier, more fulfilled lives. Some of your key responsibilities will include:

  • Personalised Care Coordination: Develop and implement individualised care plans that address the unique health needs of veterans, ensuring they receive the best possible care

  • Holistic Health Management: Monitor and manage chronic conditions, coordinate allied health & specialist referrals, and ensure continuity of care across all providers, including referrals to in-house veteran support organisations

  • Advocate for Veterans: Serve as a key advocate for veterans, ensuring their voices are heard and their healthcare needs are met

Practice Nurse - In Clinic CVC Program

  • Support and Educate: Provide education and support to veterans and their families, empowering them with the knowledge to manage their health effectively

  • Collaborate Across Teams: Work closely with the multidisciplinary team to ensure seamless coordination of care, enhancing the overall healthcare experience for our veterans

Things that will set you up for success:

  • Registered Nurse (RN) with current AHPRA registration

  • Australian work rights are required for this position

  • Experience with Best Practice software (highly regarded)

  • 2-3 years+ General Practice nursing experience (highly regarded)

  • Chronic Disease Management (highly desirable)

  • Excellent communication and interpersonal skills, with the ability to work effectively in a multidisciplinary team

  • Strong organisational and time-management skills, with the ability to prioritise tasks and manage competing demands

  • A commitment to delivering gold standard patient-centred care and a passion for improving patient outcomes

  • A true team player who thrives in collaborative environments and is eager to contribute to a purpose-driven team committed to making a difference

  • An understanding of Medicare and DVA (highly regarded)
