About the Role
We are seeking a motivated and organised Coordinator of Supports, or a suitably experienced professional, to join our team in a full-time work-from-home role.
In this role, you will work alongside participants to implement their NDIS plans, coordinate services and support participants to navigate systems and access the supports they need.
The role involves working with participants who may be navigating multiple service systems and requires strong organisation, communication and problem-solving skills.
While the position is primarily remote, there may be occasions where local meetings with participants or providers are required.

Key Responsibilities
• Supporting participants to implement their NDIS plans
• Assisting participants to identify and engage appropriate service providers
• Coordinating supports across informal, community, mainstream and funded services
• Supporting participants to build capacity and increase independence
• Assisting participants to prepare for NDIS plan reviews
• Monitoring plan utilisation and supporting participants to use funding effectively
• Navigating and coordinating supports across multiple service systems where required
• Maintaining clear, accurate and timely documentation
• Responding to challenges and problem-solving when barriers arise
• Maintaining professional and ethical practice when supporting people with disability
About You
We are looking for someone who is organised, thoughtful and motivated to support people to navigate systems and access the supports they need.
You may already have experience as a Support Coordinator, or you may bring relevant experience from the disability, community services, health, housing, mental health or broader human services sector.
The role requires someone who can think critically, communicate clearly and remain calm and solution-focused when navigating complex situations.
The successful applicant will demonstrate:
• Experience working within disability, community services, health, housing, mental health or related sectors
• Strong organisational and administrative skills
• Excellent written and verbal communication
• Confidence managing competing priorities and problem solving when challenges arise
• Strong computer literacy and the ability to maintain clear and accurate documentation
• A professional, respectful and participant-focused approach
Experience in NDIS Support Coordination is beneficial but not essential. We welcome applicants who bring experience supporting people to navigate complex service systems and access practical, coordinated supports.
Essential Requirements
• Diploma or higher qualification in Community Services, Disability, Mental Health, Social Work or a related field
• Completion of the NDIS Worker Orientation Module – Quality, Safety and You
• Current Driver’s Licence
• Working With Children Check (NSW)
• NDIS Worker Screening Check (or willingness to obtain)
