About the Role
On‑Call Coordinator
As an On‑Call Support Worker, you will work remotely to support staff and participants after hours. This includes responding to urgent roster gaps, behavioural escalations, medical concerns, and crisis situations. You’ll guide support workers through incidents, liaise with emergency services if needed, and ensure safety and continuity of care.
Shifts include:
Weekdays: 4:00pm–8:00am (overnight)
Weekends & Public Holidays (Day shift and overnight)
Flexible shift structures and extended shifts are available for additional earning potential.
Responsibilities include:
Supporting staff over the phone through behavioural, medical, or environmental incidents
Interpreting Behaviour Support Plans, Participant Profiles, and safety plans
Troubleshooting roster changes and filling urgent shift vacancies
Providing calm, therapeutic, solutions‑focused guidance
Escalating risks and liaising with police/ambulance when required
Maintaining accurate documentation and handover updates
Please Note: Sunflower Services experiences a high volume of calls, and as such, maintaining a full-time role alongside this position may not be feasible.

As a Support Worker, you will deliver person‑centred, strengths‑based support to participants with a wide range of needs. You’ll assist with daily living, community participation, skill development, emotional support, and engagement in meaningful activities.
Responsibilities include:
Providing high‑quality, trauma‑informed support to participants
Assisting with daily living, personal care (where required), and community engagement
Implementing Behaviour Support Plans and following safety strategies
Building strong relationships with participants and their support networks
Ensuring accurate documentation and communication with the team
About You
Experience supporting people with disability, mental health needs, or complex presentations
Experience in crisis response, coordination, rostering, or leadership is highly valued
Strong communication and de‑escalation skills
A calm, grounded, and professional approach in difficult situations
Confidence following BSPs, safety plans, and clinical guidance
The ability to work both independently and as part of a team
Certificate III/IV in Disability, Mental Health, Community Services, or equivalent (or working toward) desirable
Ability to work after hours
