The Role
In this rewarding role, you’ll deliver high-quality, person-centred care coordination that focuses on wellness, reablement, and participant choice.
You’ll work directly with participants in their homes, developing care plans, coordinating services, managing budgets, and monitoring outcomes, all while building trusted relationships and helping people achieve their goals.
This role does also include providing support to participants on the End of Life (EOL), Assistive Technology and Home Modification pathways.
Key Responsibilities
- Conduct home visits, assessments, and ongoing care management
- Develop and review care plans, service agreements, and budgets
- Coordinate services with aged care workers, clinical partners, and providers
- Monitor changing needs, risks, and service quality
- Support participants to navigate the aged care system, including My Aged Care
- Maintain accurate documentation and ensure compliance with quality standards

About You
- Strong IT skills, including proficiency in Microsoft Word, Excel, and client management systems
- A Diploma in Community Services or Care Management is preferred; however, a Certificate IV qualification will also be considered
- A strong knowledge of aged care funding, Support at Home / CDC guidelines, and quality standards
- Some care management and care coordination experience is desirable
- Sound knowledge of aged care quality standards
- Experience working within client budgets and achieving service delivery outcomes.
- You are professional and collaborative in your approach, with a strong commitment to working as part of a team. You bring excellent attention to detail, are highly self-motivated, and thrive in a supportive, with a focus on empowering choice.
